P-THER-04: Outcome of endoscopic ultrasound-guided choledochoduodenostomy as rescue treatment after failed endoscopic retrograde cholangiopancreatography in mid and lower end common bile duct block

چکیده
S27 ENDOSCOPIC ULTRASOUND / VOLUME 6 / SUPPLEMENT 1 / AUGUST 2017 bifurcation, stricture, and papilla, followed by guidewire retrieval with snare into the duodenoscope channel and stent placement). Results: All trainees could complete the requisite steps of EUS-BD and EUS-RV in a mean time of 11 min (8–18 min). Twenty trainees had technical issues during the procedures, which were corrected by the expert (wrong scope position 4, incorrect duct puncture 8, guidewire manipulation 8). At 10 days follow-up, 9 of 28 trainees had successfully performed three EUS-RV and seven EUS-BD procedures independently at their respective institutes. Conclusions: The Mumbai EUS II hybrid model gives real-life feel of situations encountered during EUS-RV and EUS-BD. DOI: 10.4103/2303-9027.212289

Endoscopic ultrasound-guided choledochoduodenostomy with novel use of contrast- enhanced harmonic imaging
Since it was first described in 2001 [1], endoscopic ultrasound-guided choledochoduodenostomy (EUS-CDS) has been increasingly used as an alternative for biliary decompression after failed endoscopic retrograde cholangiopancreatography (ERCP). EUS-CDS has an overall technical success rate of more than 90% [2, 3], but puncturing the common bile duct (CBD) can be challenging in patients with hemob...

متن کاملEndoscopic ultrasonographic access and drainage of the common bile duct.
Endoscopic retrograde cholangiopancreatography (ERCP) is currently the standard of care for biliary drainage. In the hands of experienced endoscopists, conventional ERCP has a failed cannulation rate of 3% to 5%. Failures have traditionally been referred for either percutaneous transhepatic biliary drainage (PTBD) or surgery. Both PTBD and surgery have higher than desirable complication rates. ...

Endoscopic ultrasound (EUS)-guided biliary drainage via either gastrohepatostomy or choledochoduodenostomy are accepted and widespread procedures when endoscopic retrograde cholangiopancreatography (ERCP) fails. EUS guidewire insertion through a 19-G needle into a dilated biliary system is usually a straightforward maneuver. The drainage difficulty begins when the enterobiliary tract has to be ...
